You are trying to re-discover a recently removed/re-added target.
After a successfull discovery, you try to promote the target but this fails with this error message on the console:
ORA-06512: at "SYSMAN.EM_TARGET", line 3559

You recently removed then re-discovered the host.
You renamed a target from A to B and tried to add A as a target name.
The target exists in auto-discovery results
The target was discovered, but then discovery was cancelled (for example, with the intention to save as a different name)
