EM 12c: OVM Manager Metric Collection Error or Registration not working after upgrading to Enterprise Manager 12c Release 2 (Doc ID 1490283.1)

Symptoms

There are 2 scenarios where the OVM Manager registration may be not be working, in both cases this only happens with OVM 3.1.1 (or greater).
It does not happen with OVM 3.0.2 / 3.0.3.

After new installation of Enterprise Manager 12c Release 2 OVM Manager registration may fail.

Upgrading an Enterprise Manager 12c system to Release 2

A Metric Collection Error is reported for the OVM Manager target (and all its "sub targets") after upgrading Enterprise Manager to Release 2.
