12c Running post patch install script fails with EM-04041: Error occurred during Metadata registration (Doc ID 1503387.1)

Symptoms

 While running the post install instructions from the patch readme

5. Run the following commands -
$ $ORACLE_HOME/bin/emctl register oms metadata -service procedures -file_list proceduresList -pluginId oracle.sysman.db
$ $ORACLE_HOME/bin/emctl register oms metadata -service swlib -file $PLUGIN_HOME/metadata/swlib -pluginId oracle.sysman.db

The following error is shown:

./emctl register oms metadata -service procedures -file_list proceduresList -pluginId oracle.sysman.db

The error in the emctl.log file is
