BI Publisher Integration into EM 12c fails to connect over HTTPS to server (Doc ID 1553244.1)

After completing the BI Publisher Integration (second attempt) per the instructions in Note 1392921.1, it still fails to display reports and shows errors in bipca_.log:
java.net.ConnectException: Tried all: 1 addresses, but could not connect over HTTPS to server: port: 9702

Symptoms

Followed installation steps and the end of the configureBIP set returns and error and you cannot use BI Publisher or see any reports.

oracle.sysman.sdkImpl.core.ip.model.bip.BIPRegistrationException: Warning: BI Publisher has been registered for use with Enterprise Manager.  However, logging into the Web Application "https://<hostname.domain>:9702/xmlpserver" has failed.

However, when navigating to 'Enterprise' > 'Reports' > 'BI Publisher Enterprise Reports' you see only a partial page:
