12c Cloud Control: 12.1.0.3.0 Agent Silent Install Fails due to Missing oracle.sysman.oh monitoring plug-in (Doc ID 1584094.1)

Symptoms

Attempting to install 12.1.0.3 Agent on Windows 64 bit server by the silent installation method using the agentdeploy.bat script via a response file.
Install fails with:

INFO: oracle.sysman.top.agent:---------------------------------------------------------------
ALLVARLIG: oracle.sysman.top.agent:ERROR: The Management Agent configuration failed. The plug-in configuration for the oracle.sysman.oh monitoring plug-in may have failed, or this plug-in may not be present in the Management Agent software. Ensure that the Management Agent software has the oracle.sysman.oh monitoring plug-in, if not then retry the operation. If the agent software has the oracle.sysman.oh monitoring plug-in, view the plug-in deployment log C:\oracle\app\product\agent\agent_inst\install\logs to check if the plug-in configuration for the oracle.sysman.oh monitoring plug-in failed.
ALLVARLIG: oracle.sysman.top.agent:Agent configuration has failed

-  The Agent software was downloaded from the Self-Update page in the Console UI using the command:

emcli export_update -id=FBAD418AC41793150E2E04831AB9DB63 -omslocal -dir=<dirname>

-  There is direct internet connectivity to the My Oracle Support portal and the Software Library is also accessible to the OMS.
