The server, where the 12c Agent is installed, is rebooted abnormally (switching off the power button) without shutting down the agent. Once the Agent starts up automatically, it gets blocked and cannot upload to the OMS.
For the Agent -> OMS communication to work fine again, Agent has to be re-synchronised from the Console UI using the steps in:

This issue occurs intermittently and is not always reproducible for all Agent installations.
Host on which the Agent is installed, had a failure or was abnormally rebooted.
