Problem - KPI Alerts are not Forwarded to Enterprise Manager Incident Manager

This symptom was reported with the following versions of each component involved. The RUEI version is of most importance as that is where the issue was found.
EM Version: 126.96.36.199
FMW Plugin Version: 188.8.131.52
RUEI Version: 184.108.40.206
RUEI KPI Alerts do not show in the EM Incident Manager, event though:
- The database connection is functional
- The KPI do trigger alerts and e-mails are send from RUEI
- The KPI is presented on the Business Application Dashboard.
