12c EM: Management Services and Repository Metrics - "Long Running Metric Collection Status" And "Short Running Metric Collection Status" are Shown as PARTIALLY UP

Symptoms
In the Setup -> Manage Cloud Control -> 'Health Overview' page of the 12c Console, the below metrics:
Long running metric collection status
Short running metric collection status
shown as 'PARTIALLY UP' with a warning severity.
- Patches listed in <Note 1907579.1> : EM 12.1.0.4: Invalid Status (Partially UP or DOWN) for Long Running Metric Collection Status and Short Running Metric Collection Status, have been applied to the OMS and Agent already.
- Installing the EM Diagkit (repvfy) from <Note 1426973.1> and executing the command "repvfy verify -level 9" reports:
verifyREPOSITORY
7004. Obsolete worker trace parameters: 203
7007. Obsolete worker thread status: 178
- The output from "repvfy dump task_health" reports:
