OATS: Oracle Test Manager Cannot Be Accessed after Database Schema Change (Doc ID 2045235.1)

Symptoms

The database schema password was changed for the schema being used by Oracle Application Testing Suite (OATS) Oracle Test Manager (OTM), which results in a 404 Error in the browser and the following error message in the wls_oats.log file.
