Symptoms

 On rare occasions, upgrading a multi-OMS system from EM 12.1.0.3 to 12.1.0.5, where the integrated BI Publisher is configured with BIP Shared Storage, i.e. 'emctl config oms -bip_shared_storage -config_volume <vol1> -cluster_volume <vol2>', the BI Publisher Server fails to start with the following errors:

Changes

EM 12c from 12.1.0.3.0 (PS2) upgraded to 12.1.0.5 (PS4), but this may not be related to the actual cause.
