OATS: 'Error 404--Not Found' when trying to access OLT/OTM Server (Doc ID 2103919.1)

Goal

When bringing up Oracle Load Testing (OLT) or Oracle Test Manager (OTM) you get the following error reported in the browser.

Error 404--Not Found
From RFC 2068 Hypertext Transfer Protocol -- HTTP/1.1:
10.4.5 404 Not Found

The following error message appears in the log file:

<OATS_INSTALL>/logs/wls_oats.log:
