EM 12c : Incident Rule Simulation / Invocation Fails with to "Unable to Apply Rules on this Objects, error occurred during assertion" Error (Doc ID 2143877.1)

Symptoms

For EM events that should be creating incidents or sending notifications, OMS is not invoking any incident rule.
Verifying the Event annotation (Notifications or All Updates tabs) shows the message : "unable to apply rules on this objects, error occurred during assertion"
Or "Updated state by skipping rule processing"


-  This issue affects all Events and no Incident rules are being invoked by the OMS.
-  There are no broken incident rules or incident rulesets.
-  The <gc_inst>/em/EMGC_OMS1/sysman/log/emoms_pbs.trc shows exceptions such as below:
