EM 12c: Database Upgrade From 11.2.0.4 to 12.1.0.2.160419 from Enterprise Manager 12c Cloud Control Fails with Error: emrconfig exited with 256

Symptoms

In Enterprise Manager (EM) 12.1.0.5 Cloud Control, database upgrade from 11.2.0.4 to 12.1.0.2.160419 fails with the following error (this issue happens for a database on an Oracle Restart server):
