Upgrade 12c Cloud Control to 13c Cloud Control.
Login to 13c Cloud Control as sysman user and navigate to Enterprise > Monitoring > Corrective Actions.
In this page, clicking in link available near 'Broken Corrective Actions' shows a lot of corrective actions broken due to reason as 'No Credentials'.
Later From Cloud Control navigate to Enterprise > Monitoring > Corrective Actions.
Click link near 'Broken Corrective Actions'.
select Corrective Action for target, and click 'Edit' button.
select Named credentials, and click OK, which fails with : ORA-01422
