Patch Plan 'Review & Deploy' Tab Shows Issues Even Though Procedure Activity Succeeds (Doc ID 2265167.1)

Symptoms

From Enterprise Manager Create Patch plan for database or Grid Infrastructure by navigating to Enterprise > Provisioning and Patching > Patches & Updates.
Search for a patch and add needed targets to Patch Plan and do Analyze of the Patch plan.
Navigate to Enterprise > Provisioning and Patching > Procedure Activity and validating result of Patch Plan shows Success.
But navigating to 'Review & Deploy' tab of Patch Plan shows that there is a conflict with previous PSU or Combo patch.
