EM 12c: Enterprise Manager 12c Cloud Control Database Target Monitoring Configuration Properties: No Data To Display (Doc ID 2266833.1)

Symptoms

A upgraded database was an existing target in Enterprise Manager (EM) 12c Cloud Control 12c monitoring.  
The Monitoring Configuration must now be changed to reflect the new Oracle Home value.
Navigating to the target database home page reveals a message stating that the DBSNMP account is locked.
An attempt to modify the DBSNMP password by accessing the page that should contain the text boxes to input the DBSNMP password and Oracle Home values are missing.  
Instead, in its place, it shows
