"emrconfig exited with 256 at" errors in DB Creation step while selecting different Gold image versions for DB and Oracle restart (Doc ID 2270949.1)

Enterprise Manager for Oracle Database - Version 12.1.0.8.0 to 13.2.2.0.0 [Release 12.1 to 13.2]

Symptoms

From 13.2 Enterprise Manager navigate to Enterprise > Provisioning and Patching > Database Provisioning.

Launch 'Provision Oracle Database' Deployment Procedure.

in 'Select software locations', selected 12.2 Oracle Restart image for 'Grid Infrastructure' and selected 12.1.0.2 database image for Oracle Database.

The Provisioning Activity Page failed at 'Create Database' step with following message:
