EM 13c : After Upgrading Agent from 12c to 13.1, its Status is Shown as "Availability Evaluation Error" in the Console UI

Information in this document applies to any platform.
The agent was recently upgraded from 12c to 13.1 release.
After the upgrade, the agent is shown with "availability evaluation error" status in the console UI. The status is however marked as UP sometimes but reverts back to an error after sometime.
The agent however is up and running fine on the target host.
The <AGENT_INST>/sysman/log/targets/oracle_emd/<agent_target>/SEG_*.log file shows:
