EM 12 13c: Database Home page shows Incorrect DB version ,uptime

Target database has been upgraded from 22.214.171.124.2 to 126.96.36.199.4.
Navigating to DB home page (Target > Database > click on Database link) show
a. Incorrect database version for databases upgraded from 188.8.131.52.2 to 184.108.40.206.4
b. Incorrect Uptime
DB home page
EM uses following query to show version and uptime.

"union all \n" +
