EM 12c: Enterprise Manager 12c Cloud Control Target Status is Agent Unreachable but the Agent is Running (Doc ID 2308368.1)

Symptoms

Enterprise Manager (EM) 12.1.0.5.0 Cloud Control on Linux x86-64

After a network interruption (via WAN), EM 12.1.0.5 reports that a few agents are in Agent Unreachable status.
Agents are up and running on each server.
The commands pingOMS and upload are successful.

OMS host and Agent host network connectivity is also successful via ping, nslookup, telnet and wget commands.
