EM 13c, 12c: Console Login with A Specific Administrator Fails with "Authentication failed. If problem persists, contact your system administrator"

Symptoms

Console login with one particular user gives the following error:

 "Authentication failed. If problem persists, contact your system administrator"

SYSMAN and other users are able to login to Console without any issues.
