OEM 13c: PDB Prefix From Service Template Is Not Being Applied When PDB is Provisioned From Self Service Portal (Doc ID 2421400.1)

Symptoms

 Suppose a PDB Service Template is created with an SID Prefix of SC19. When a PDB is requested with SID ORCL from the Self Service portal, the PDB created should be of SID SC19ORCL, instead it is created as just ORCL.
