EM 13c: Incorrect Target Type shown for Dataguard or Primary Database After Switchover

Symptoms
In EM 13c, After switch over happens either primary or physical standby targets are shown incorrect target type.
Example: Both databases are shown as primary or Both are shown as Standby Database on Targets > Databases page (TYPE Column on this page).
In Target Database > Availability > Data Guard Administration database status is displayed correctly.
