EM 13c, 12c: Database / RAC Database Targets are Stuck in "Availability Evaluation Error" in EM Console (Doc ID 2467557.1)

Symptoms

In EM console, database target / RAC Database targets are stuck in "Availability Evaluation Error" though the targets and the agent monitoring these targets are UP.
Execution of getmetric as below gives an error stating the target does not exist.

$emctl getmetric agent <targetName>,oracle_database,Response
