EM 12c, 13c: EM Console shows old version after WLS Target is Upgraded (Doc ID 2526040.1)

After upgrading a WebLogic Server (WLS) target, the Enterprise Manager (EM) Console Home Page for this target is showing the wrong WLS version.

The WebLogic Administrative Console shows this target's AdminServer is version 12.2.1.3.0:

which is confirmed in <domain home>/config/config.xml:

but this target's Home Page in the EM Console shows version 12.1.3.0.0 (the version before WLS upgrade):

The WebLogic Server target was upgraded using an upgrade installer.
