From EM 13c Console, following error is seen on every database target home page:

Regions that display real-time data will not be displayed. Exception while loading RAC Database Home Page: IO Error: The Network Adapter could not establish the connection The Connect Descriptor was (DESCRIPTION = (ADDRESS_LIST = (ADDRESS = (PROTOCOL = TCP)(HOST = <HostName>)(PORT = 1521))(ADDRESS = (PROTOCOL = TCP)(HOST = <HostName>)(PORT = 1521)))(CONNECT_DATA = (SERVICE_NAME = <ServiceName>)))

Test connection from Oracle Database > Target Setup > Monitoring Configuration also fails with the same error.
