Symptoms

After upgrading a Forms and Reports production environment from 12.2.1.2 to 12.2.1.4 on Microsoft Windows x64 (64-bit) 2012 R2 the EM FMW Control keeps spinning after entering login password.
The following error is displayed in the logs files:
