OEM 13.5 When Discovering Pluggable DB: Failed To Promote Credential Change To Agent For (n) Targets

Symptoms
In Enterprise Manager (EM) Cloud Control, when attempting to discover Pluggable databases and when trying to change the password the following error occurs: "Failed to promote credential change to agent for 3 targets".
But the password was changed in the database and in the OEM configurations successfully. Also, OEM shows the database down, but it's running. Apart from this, the agent is running and uploading info.
/<INSTANCE_HOME>/em/EMGC_OMS1/sysman/log/emoms.trc shows the following error message:
This output shows that the targets have duplicated entries in the table.
