Symptoms

Holistic Patch apply failed with following error

$ORACLE_HOME/OPatch/opatch apply

OPatch version : 13.9.4.2.16
OUI version : 13.9.4.0.0
Log file location : <OMS_HOME>/cfgtoollogs/opatch/opatch<Date time stamp>_1.log

OPatch detects the Middleware Home as "<OMS_HOME>"

Verifying environment and performing prerequisite checks...
OPatch continues with these patches: 36741053

Patching component oracle.webcenter.wccore, 12.2.1.4.0...
ApplySession failed in system modification phase... 'ApplySession::apply failed: oracle.glcm.opatch.common.api.install.HomeOperationsException: A failure occurred while processing patch: 36741053'
