RAC DB Control satus reports <AGENT_HOME>/hostname.domain.com not found or inconsisten state of RAC targets in Grid Control (Doc ID 436845.1)

Symptoms

A. RAC configured envionment

RAC and its associated targets may appear as being in inconsistent states. 

The example below is referred to a RAC environment but symptons may also occur in DB Control (please see Section B) environments as well.

Note - Therefore the states across all nodes in RAC may consist of  UP, PENDING or UNREACHABLE status but is not limited to these symtpons.   
SETUP > AGENTS > <AGENT_HOSTNAME>:<AGENT_PORT>
SETUP > AGENTS > <AGENT_HOSTNAME>:<AGENT_PORT>
Login to either hosts as oracle user and verify status of agent:   
<AGENT_HOME>/bin/emctl status agent
Returns the following error on myhost1:    
EM Configuration Issue. <AGENT_HOME>/<AGENT_HOSTNAME> not found
Returns the following error on myhost2:    
EM Configuration Issue. <AGENT_HOME>/<AGENT_HOSTNAME> not found

B. DB Control configured environment

The following error may be similarly reported in a DB Control environment.

If using DB Control, the following error is reported when running:

<ORACLE_HOME>/bin/emctl status dbconsole

or:

<ORACLE_HOME>/bin/emctl status agent

Returns the following error:

EM Configuration Issue. <DB_HOME>/<AGENT_HOSTNAME>_<DB_NAME> not found

For agent that may appear as being UP, the following process may still be running in background or rogue:   

ps -ef|grep emagent
The following may appear if emagent process may still be running:                         
oracle 6328 1 0 Jun19 ? 00:00:02 <AGENT_HOME>/perl/bin/perl
<AGENT_HOME>/bin/emwd.pl agent
<AGENT_HOME>/sysman/log/emagent.nohup
oracle 6345 6328 0 Jun19 ? 00:03:13 <AGENT_HOME>/bin/emagent
oracle 8753 14862 0 01:18 pts/2 00:00:00 grep emagent
