EMCA Fails With The Error 'Failed to unlock all EM-related accounts' when Creating DBConsole (Doc ID 756076.1)

Symptoms

Creating dbconsole via Enterprise Manager Configuration Assistant (EMCA) fails with:

CONFIG: Failed to unlock mgmt_view account oracle.sysman.emcp.EMReposConfig invoke
SEVERE: Failed to unlock all EM-related accounts

The error below is also reported in emca logs

Connecting to database <SID> in home <ORACLE_HOME> with user "SYS" role "SYSDBA".
oracle.sysman.emcp.util.PlatformInterface executeCommand
CONFIG: Couldn't connect to the db. at <DATABASE_HOME>\sysman\admin\scripts\emca\emcaDbUtil.pl line 85, <STDIN>
line 2.
