FDM Error "Index Was Out of Range" in User Maintenance When Pulling Users From Shared Services (Doc ID 1065546.1)

Symptoms

When attempting to add a newly provisioned user to the list of users within "User Maintenance" in FDM, the new user screen takes a long time processing.  When the screen is returned it returns the error message:

Error: Index was out of range.  Must be non-negative and less than the size
of the collection.  Parameter name: index
