Shared Services Provisioning Fails Error "30:1101:JNDI" for FDM (Doc ID 1139713.1)

Symptoms

When attempting to login to FDM with a user who was previously provisioned and added to the user maintenance list in FDM, the login fails with the error message:

Error: An error occured logging on to the system.
Detail: The username or password is invalid.

If the user is de-provisioned and attempted to be re-provisioned a JNDI 30:1101 error is returned with a LDAP Status code of 20.  LDAP Status code of 20 states "LDAP_TYPE_OR_VALUE_EXISTS Type or value exists "
