OBIEE 11g: Error: "[nQSError: 13011] Query for Initialization Block 'Authorization' has failed." after Database Containing Repository Schemas Crashed (Doc ID 1299277.1)

Symptoms

You are unable to log into OBIEE 11g after the database containing the repository schemas crashed. There was also a change in the tnsnames information for the server where the schemas were installed.

Errors included:

1. NQServer.log:
