OBIEE 10g: Error: "Error Codes: OPR4ONWY:U9IM8TAC:OI2DL65P [nQSError:15018] Incorrectly Defined Logical Table Source" When Running a Report

When running a report  in Answers, instead of displaying the results, you see the following error:

Odbc driver returned an error (SQLExecDirectW).
Error Details
State: HY000. Code: 10058. [NQODBC][SQL_STATE:HY000][nQSError: 10058]A general error has
occurred.[nQSError: 15018] Incorrectly defined logical table source (for fact table <
fact table name>) does
not contain mapping for [<
dimension table.dimension name>].(HY000)

You might also see this error:


State: S1000. Code: 10058. [NQODBC] [SQL_STATE: S1000] [nQSError: 10058] A general error has occurred. [nQSError: 14070] Cannot find logical table source coverage for logical columns: [Source]. Please check more detailed level keys are mapped correctly. (S1000)


In some cases changing the order of the dimension columns allows the results to display without error; or creating the report with just each dimension column at a time allows the report to display without error


Possible changes prior to seeing this error are:
Upgrading to OBIEE
Adding a new table to the repository.
