OBIEE 11g: Error: "[nQSError: 43126] Authentication failed: invalid user/password" When Logging In (Doc ID 1316008.1)

Symptoms

You cannot log in to OBIEE. Nor can you open the repository in online mode. But you can connect to the repository in offline mode. You see the following error in the nQServer.log file:
