"The SQL operation failed" Error When Trying to Create New Version or Scenario Members

Symptoms
When trying to add a new member to the Scenario or Version dimension in a Planning classic application, the following error is displayed:The SQL operation failed. Check logs for details.
An example of the error in the Planning log is:
[11/17/11 11:20:25:393 EST] 00000024 SystemErr R java.sql.SQLException: [Hyperion][SQLServer JDBC Driver][SQLServer]Cannot insert the value NULL into column 'ENABLED_FOR_PM', table 'HyperionPLAN1.dbo.HSP_SCENARIO'; column does not allow nulls. INSERT fails.
The log extract above shows an attempt to add a new scenario member when using Microsoft SQL Server as the relational database.
