OBIEE 11g: Error: "[nQSError: 43126] Authentication failed: invalid user/password." When Logging in to Answers or Dashboard

Symptoms

An OBIEE 11g LDAP User will not authenticate at log in despite other LDAP users also held in the same LDAP Directory location will authenticate successfully.  In nqserver.log you see the following error:
[2011-12-11T08:23:15.000+00:00] [OracleBIServerComponent] [ERROR:1] [] [] [ecid: 00i4Q18cVHL3r23_NlO5ye0000fS000000] [tid: d80] [nQSError: 43126] Authentication failed: invalid user/password.
