Essbase Administration Services (EAS) Does Not Start, Errors with "Error Instantiating 'dc=EPMSystem': null" (Doc ID 1499240.1)

Symptoms

The EAS Server does not start.  The MIDDLEWARE_HOME/user_projects/epmsystem1/diagnostics/logs/starter/start-EssbaseAdminServices0-out.log shows the error:
