OBIEE 11g: S_NQ_DB_ACCT Usage Tracking Table is not Populated

You have configured usage tracking by following chapter 9 of the Oracle® Fusion Middleware System Administrator's Guide for Oracle Business Intelligence Enterprise Edition 11g Release 1 (11.1.1) Part Number E10541-05
Usage tracking is writing to the S_NQ_ACCT table, you can see errors in the NQServer log about it not being able to write to the S_NQ_DB_ACCT table. For example:
