OBIEE 11g: Error: "Blacklisted connection pool" Causes BI Server / NQServer to Crash (Doc ID 1684120.1)

Symptoms

When attempting to use OBIEE, the BI Server shuts down with the following error occurring in NQServer.log file:

[nQSError: 17001] Oracle Error code: 942, message: ORA-00942: table or view does not exist  at OCI call OCIStmtExecute.
