OBIEE 11g: Error: "Blacklisted connection pool" Causes BI Server / NQServer to Crash (Doc ID 1684120.1)

When attempting to use OBIEE, the BI Server shuts down with the following error occurring in NQServer.log file:

[43143] Blacklisted connection pool: '".......l"'
[OracleBIServerComponent] [ERROR:1] [] [] [ecid: <ECID>] [tid: xxxxxxxx] [nQSError: 17010] SQL statement preparation failed. [[
[nQSError: 17001] Oracle Error code: 942, message: ORA-00942: table or view does not exist  at OCI call OCIStmtExecute.


[OracleBIServerComponent] [ERROR:1] [] [] [ecid: <ECID>] [tid: xxxxxxxx] [nQSError: 17014] Could not connect to Oracle database. [[
[nQSError: 17001] Oracle Error code: 28001, message: ORA-28001: the password has expired at OCI call OCISessionBegin.
[nQSError: 43059] Init block '...': Dynamic refresh of repository scope variables has failed.
