Hyperion Planning Error "Cannot connect to the provider" When Selecting 'Open in Smart View' (Doc ID 1916531.1)

Symptoms

When attempting to open a Hyperion Planning form in Smart View using 'Open in SmartView', the following error messages occur:

Cannot connect to the provider. Make sure it is running in the specified host/port. Error (12157).

followed by:

Web launch operation has been cancelled.

The same form can be successfully opened from within SmartView.
