Hyperion Planning Error: "The Form Is Invalid" when Connecting To Ad hoc (Doc ID 1917429.1)

Symptoms

On : 11.1.2.3.500 version, Smart View

When attempting to connect to a particular plan type when doing ad hoc with either Planning or Essbase, the following error occurs.

ERROR
-----------------------
Scenario1: When performing an ad hoc analysis on a particular plan type at the Account level and connecting to Planning, we get an error "The form is invalid".

Scenario2: When performing an ad hoc analysis on a particular plan type for Planning at the Account level and connecting to Essbase, we get a different error "Essbase Error (1023040): msg from remote site [[Thu Aug 14 11:00:00 2014]
Local////Error (1051293) Login fails due to invalid login credentials]"


STEPS

The issue can be reproduced at will with the following steps:
1. Open Smart View and connect to a Planning connection.
2. Connect to the Plantype Console and perform an ad hoc analysis
3. Select Account and get error above for Scenario 1.

OR

1. Open Smart View and connect to an Essbase connection.
2. Connect to the Plantype Console and perform an ad hoc analysis
3. Select Account and get error above for Scenario 2.
