OBIEE 11g - Custom SSO Using RSA Cleartrust Accessmanager Fails and the OBIEE Sign In Page is Displayed

Symptoms

OBIEE 11g has been configured to participate in a custom SSO environment where users are authenticated via RSA Cleartrust Accessmanager (against a MS Active Directory LDAP) and an HTTP Header named "samaccountname" containing the User ID is populated.

The user navigates to the virtual URL where he is prompted for his SSO credentials.  However, after the user supplies his SSO credentials, the OBIEE Sign In page is displayed, and the user is not logged into OBIEE.
