Hyperion Planning Error When Using Offline Planning in Smart View: "XML Load Error: A Semi Colon Character Was Expected"

Symptoms

After Offline Planning has been installed successfully on client machine and creating the offline connection, opening the offline form generated the following error in Smartview:

"XML Load Error: A semi colon character was expected."
