OBIEE 11g - Unable To Deploy a RPD and Activate Changes in Enterprise Manager (EM) Due To "java.lang.StringIndexOutOfBoundsException" Error

In FMW Enterprise Manager (EM), upload a new RPD file and click Apply; then click Activate and see the following error is thrown:

Activate Changes - Completed Successfully
Commit failed with 0 errors and 2 warnings.  Problems reported by: bi_server1.  See detailed logs for more information.

In the AdminServer-diagnostic.log file, you see messages confirming that the new RPD was packaged and propagated without error; but then a timeout occurred while waiting for the changes to be applied to the Managed Server:

Processing Event: WARNING: Element Type: DOMAIN, Element Id: null, Operation Result: FAILED_TO_APPLY_CHANGES,
Detail Message: Timeout; Waiting for all managed servers to push configuration changes; [180 secs]; bi_server1
