[EPMFM] [ERROR:16] is a generic error message is commonly found in conjunction with other messages during the time of an HFM crash. Typically, users are either unable to access HFM from Workspace OR they are kicked out while accessing the application. It is helpful to review the log entries before and after the time of the crash to help determine the cause of the issue.
