OBIEE 11g|12c: Agents Send Emails To Incorrect Recipients When Primary Trigger Agent Is Present

There is a random event noticed by users that their agents deliver incorrect report to members of other child agents. Upon further investigation they have been able to determine from Job Manager Tables that incorrect recipients have indeed been sent email intended for other recipients. This has only seen to happen when there is primary agent present and there is trigger or chain concept.
Scenario Example (10 is just an example):
Primary Agents has 10 Child Agents
Each Child Agent has 10 Email Recipients
All 10 Child Agents run in parallel
There have been 2 symptoms which have been consistent in all cases that have happened so far:
1. select * from S_NQ_ERR_MSG where INSTANCE_ID=<incorrect delivery instance number> --> when compared with delivery list as seen on Agent "Recipients" tab --> you will observe a mis-match
2. Agent Log for the instance which reported incorrect delivery will not be generated unless debug option is set to true for Agent Logs
No changes have been made.
