OBIEE 11g: Usage Tracking not Started due to Non-existent Usage Tracking Table (Doc ID 2139634.1)

Symptoms

Usage Tracking has been set up, however Usage Tracking data does not gather in S_NQ_ACCT when executing analysis.

The following error is logged in nqserver.log when starting Oracle BI Server.
