OBIEE 12c: OBIEE 12c Install with OHS results in HTTP 404 error

Symptoms

 Perform a fresh installation of OBIEE and configure OHS. weblogic plugin is enabled for the managed server. lightweightSSO parameter is also disabled in the EM. After the installation of OBIEE and configuring the OHS, it is possible to login to EM and Console from OHS. Rest of the URLs are not working and showing 404 error. Logging in to Analytics URL the following error is produced
